Clinical performance of magnetic resonance imaging and biomarkers for prostate cancer diagnosis in men at high genetic risk

The study evaluated 322 patients with high genetic risk of PCa and assessed different diagnostic markers and scenarios for managing early diagnosis of cancer. The results indicate that mpMRI with a PI-RADS score ≥3 is more sensitive for detecting PCa and clinically significant PCa compared to PSA level screening. Pre-screening with PSA level may result in missing high-grade tumors and unnecessary biopsies. In cases of negative mpMRI, the study suggests using urinary markers or PSAD of ≥0.10 ng/mL/mL to increase the detection of high-grade cancers. The study recommends a baseline mpMRI for men at high genetic risk from the age of 40 years.
