Cognitive impairment is common in CKD, driven by factors like uraemic toxin buildup, microvascular damage, inflammation, and accelerated ageing. Advances in imaging and longitudinal studies are uncovering mechanisms such as blood–brain barrier disruption and endothelial dysfunction. Understanding these pathways offers new opportunities to treat CKD-related cognitive decline and address cognitive dysfunction in broader ageing populations.
