Combined Biopsy Method Improves Prostate Cancer Detection Rates

In a retrospective analysis of 216 patients, MRI-targeted biopsy (MRI-TB) combined with systematic biopsy (SB) detected clinically significant prostate cancer (csPCa) at higher rates than MRI-TB alone. csPCa was identified in 61.1% of patients using MRI-TB with systematic transrectal/transperineal biopsy (sTR/TP-bx) and in 56.0% with MRI-TB and sTP-bx, compared to 46.8% with MRI-TB alone. Risk factors associated with higher csPCa detection included elevated PSA density (PSAD), PI-RADS score ≥4, smaller prostate volume, peripheral zone location, and dutasteride use. A scoring system based on PSAD, PI-RADS, and dutasteride effectively stratified detection likelihood, with high-risk groups showing detection rates of 95.5% and 97.8% in MRI-TB with sTR/TP-bx and sTP-bx, respectively. This combined biopsy approach may optimize csPCa detection, especially in high-risk patients.
