
Combination treatments for migraine prophylaxis hold promise in addressing the complex mechanisms of migraine. This review evaluates the potential of combining oral prophylactics, onabotulinumtoxin A, CGRP monoclonal antibodies (mAbs), and CGRP receptor antagonists (gepants). Dual CGRP inhibition via mAbs and gepants may offer enhanced efficacy, while onabotulinumtoxin A combined with CGRP-targeted therapies demonstrates synergistic benefits. Oral non-CGRP prophylactics remain practical for patients with comorbidities. However, limited evidence and high costs hinder widespread clinical adoption. Large-scale trials are essential to define safe, effective protocols for these combinations, especially in treatment-resistant cases.
