
This study assessed the effectiveness and safety of percutaneous endoscopic interlaminar discectomy (PEID) for treating deeply migrated disc herniation. A retrospective analysis of 39 patients who underwent PEID between January 2015 and October 2020 was conducted. Results were compared with 37 patients treated with posterior lumbar interbody fusion (PLIF). PEID showed shorter operation times (93 mins vs. 169 mins for PLIF) and comparable improvement in pain (VAS) and disability (ODI) scores. Recurrent herniation was managed effectively with repeat PEID, and complications were manageable. The modified MacNab criteria indicated favorable outcomes for both procedures (PEID: 97.43%, PLIF: 94.60%). PEID is a reliable and safe option for highly downward-migrated disc herniation treatment, with comparable long-term efficacy to PLIF.
