The study aimed to compare real-world outcomes of older patients with atrial fibrillation (AF) treated with anticoagulation versus left atrial appendage occlusion (LAAO) by sex, as there may be differences in clinical trial outcomes. The study used Medicare claims data from 2015 to 2019 and found that LAAO was associated with a significant reduction in the risk of mortality and stroke or systemic embolism for both women and men, compared to anticoagulation alone. The study also found that bleeding risk was higher early after LAAO implantation but lower after the 6-week periprocedural period for both women and men. These findings suggest that LAAO may be a beneficial alternative to anticoagulation for reducing AF-related stroke in older patients and should be considered in shared decision-making with patients.
