Fontan circulation is associated with a risk of late mortality from cardiac and noncardiac causes. Referral indications for advanced heart failure care vary between centers. Many individuals die from complications after late consideration for advanced heart failure therapies or without seeing a specialist. The Advanced Cardiac Therapies Improving Outcomes Network (ACTION) Fontan Committee has developed guidelines for appropriately timed referral for advanced heart failure consultation, divided into four categories of clinical Fontan circulatory dysfunction. These guidelines aim to guide primary cardiologists in referring patients for advanced heart failure care.
