COOL-AF Model Improves ICH Risk Prediction in Atrial Fibrillation

A prospective nationwide registry in Thailand analyzed 3405 patients with non-valvular atrial fibrillation (AF) to identify intracranial hemorrhage (ICH) predictors. Over an average follow-up of 31.8 months, ICH occurred in 70 patients (2.06%), with an incidence rate of 0.78 (0.61−0.98) per 100 person-years. Significant predictors included older age, male sex, nonsmoking, renal replacement therapy, and oral anticoagulant use. The COOL-AF model for predicting ICH risk demonstrated superior accuracy compared to the HAS-BLED score, with a C-index of 0.717.
