Cost-Effectiveness of Modified Diagnostic Strategy for Pulmonary Embolism in Emergency Departments

The trial-based economic evaluation compares the modified diagnostic strategy, combining the YEARS rule and age-adjusted D-dimer threshold, with the control using only the age-adjusted D-dimer threshold for diagnosing pulmonary embolism (PE) in Emergency Departments (EDs). Conducted in 16 French and two Spanish EDs, the study analyzes 1,217 patients. At three months, no significant cost differences were found (€-46; 95% CI: €-93 to €0.2), and the intervention group's failure rate was non-inferior. The incremental cost-effectiveness ratio (ICER) suggests potential cost savings (€7,142 per undetected VTE averted). Policy-makers are encouraged to explore how these savings can be distributed among stakeholders.
