A study comparing oral cyclophosphamide (CYC) and mycophenolate mofetil (MMF) for treating steroid-sensitive nephrotic syndrome in children found similar efficacy for both drugs in terms of response rates. In the CYC group, 50% of patients were responders, while 54% of MMF patients met the response criteria (p = 0.614). The time to first relapse was similar for both treatments, with CYC and MMF showing 7 months (IQR 5.25–11) and 7 months (IQR 3.5–12) respectively (p = 0.092). However, the relapse rate was significantly lower in the MMF group (1.295 relapses per patient-year) compared to the CYC group (1.77 relapses per patient-year) (p = 0.009). Younger age at treatment initiation was associated with better response to MMF (p = 0.039). Overall, MMF demonstrated comparable efficacy to CYC with fewer relapses and favorable treatment outcomes.

Chronic urticaria is a heterogeneous mast cell-driven disease with major effects on quality of life. Current care emphasizes diagnosis, limited targeted testing, disease monitoring and stepwise treatment, beginning with second-generation H1-antihistamines and up-dosing when needed. Omalizumab and newer therapies such as dupilumab and remibrutinib expand options for refractory disease, while future care may be guided by biomarkers and disease endotypes.

A retrospective study of 33 women with postpartum hemorrhage during cesarean delivery found that uterine compression sutures achieved hemostasis in 69.7% of cases. Suspected placenta accreta spectrum was significantly more common among women needing escalation (60.0% vs. 8.7%; p=0.004).

PULMONOLOGY

ICS–SABA reduced asthma exacerbations and corticosteroid use without increasing adverse events in a five-RCT meta-analysis.

A meta-analysis of five randomized controlled trials found that as-needed ICS–SABA significantly reduced asthma exacerbations compared with SABA-only reliever therapy. ICS–SABA lowered exacerbation rates and the risk of experiencing at least one exacerbation by 31%, while extending time to first exacerbation and reducing systemic corticosteroid exposure by about 39 mg, without increasing adverse events.

An exploratory study found that the teach-back method, supported by multimodal communication strategies, improved understanding during a simulated informed-consent process among people with aphasia.
