The study investigated the relationship between endothelin-1 (ET-1) levels, kidney function, and outcomes in heart failure patients receiving dapagliflozin, a medication used to treat heart failure. Results showed that higher baseline ET-1 levels were independently associated with worse clinical outcomes and a more rapid decline in kidney function. However, dapagliflozin benefits patients across all ET-1 concentration levels, with a small decrease in serum ET-1 concentration observed with dapagliflozin treatment. These findings highlight the prognostic importance of ET-1 in heart failure and renal disease and suggest that dapagliflozin may be effective regardless of baseline ET-1 levels.
