To study the association between moderate or greater depression development during curative‐intent therapy and overall survival (OS) in patients with stages II‐IV head and neck cancer (HNC). During HNC treatment, clinically significant depression developed in 22 of 125 patients (17.6%). The mean follow‐up was 5.0 years (SD 2.4). OS was similar for patient groups when stratified by the development of moderate or greater depression or the use of prophylactic antidepressants. Prophylactic antidepressants may be considered in patients with HNC to prevent clinically significant depression and offer improved quality of life outcomes.
