Decongestion Strategies in patients presenting with acutely decompensated heart failure: a worldwide survey among physicians

A global survey involving 641 physicians from 60 countries explored the treatment strategies for acute decompensated heart failure (ADHF) with a focus on differences between cardiologists and non-cardiologists. The survey highlighted significant variations in approaches, such as limited use of diuretics (only 7% administered high-dose intravenous furosemide), infrequent ultrasound assessment for congestion (20.4%), and rare measurement of spot urinary sodium. Around a third of respondents considered ongoing edema as stabilization. Cardiologists and non-cardiologists differed in managing ADHF, including targets for weight loss and urine output, diuretic escalation strategies, and assessment of treatment response. This study emphasizes diverse practices in ADHF management.
