
The study aimed to enhance diagnostic accuracy for rapidly progressive dementia (RPD) by utilizing cerebrospinal fluid (CSF) biomarkers. The study findings revealed that Alzheimer's disease biomarkers were linked to neurodegenerative RPD, while elevated NfL, sTREM2, YKL-40, and decreased VILIP-1 were associated with autoimmune/inflammatory RPD. Vascular RPD was characterized by increased MCP-1 levels. A predictive model involving GFAP, MCP-1, p-tau181, and sTREM2 accurately identified patients with treatment-responsive RPD. However, distinctions between typical and rapidly progressive neurodegenerative diseases were minimal. Overall, the selected CSF biomarkers demonstrated associations with etiologic diagnoses and treatment responsiveness, but their ability to elucidate mechanisms driving rapidly progressive neurodegenerative disease remains unclear.
