Diastolic Exercise Stress Testing in Heart Failure with Preserved Ejection Fraction – The DEST HF study

A prospective study on intermediate heart failure risk patients, researchers examined different diastolic stress tests to detect hidden heart failure with preserved ejection fraction. Nineteen patients were randomly assigned to undergo four stress tests: leg raise, fluid challenge, handgrip, and bicycle ergometry. Results revealed significant increases in exercise-induced pulmonary capillary wedge pressure (PAWP) across all tests compared to rest, with bicycle ergometry showing the highest increase (Δ22.3±5.0mmHg). Bicycle ergometry also led to notable changes in pulmonary artery pressure, cardiac output, and related ratios. Notably, achieving PAWP≥25mmHg was reliably feasible through bicycle ergometry, underscoring its importance in spotting exercise-induced PAWP elevation.
