This systematic review and meta-analysis examined the efficacy of drug-induced sleep endoscopy (DISE)-directed tongue surgery in children with persistent or recurrent obstructive sleep apnea (OSA) after adenotonsillectomy. The analysis of seven studies (283 patients) showed that the surgery resulted in a 50% reduction in the apnea-hypopnea index (AHI) and a 3% improvement in the lowest oxygen saturation (LSAT). The overall surgical response rate was 70%. Although DISE-directed surgery appears effective, there is notable heterogeneity in reported outcomes, suggesting confounding factors may influence results.
