
A recent study evaluated 110 consecutive head and neck reconstructions utilizing the pectoralis major myocutaneous flap (PMF). The study revealed that skin paddle necrosis was rare when the extension (l2) was within the range of 2.0–3.0 cm, occurring in just 1% of cases (with one exception involving an obese patient). However, when the extension exceeded 3.5 cm, necrosis was observed in 16% of subjects, mainly when the proximal extent (l1) was less than 67% of the entire skin paddle. Careful patient selection and adequate proximal SP were also associated with improved outcomes.
