Downstream Biomarker Effects of Gantenerumab or Solanezumab in Dominantly Inherited Alzheimer Disease

A study on dominantly inherited Alzheimer's disease (DIAD) examines the impact of anti-amyloid treatments on biomarkers in cerebrospinal fluid (CSF) and plasma. Out of 236 participants screened, 142 were included. Gantenerumab significantly reduced CSF neurogranin levels and plasma GFAP levels, while increasing CSF sTREM2 levels. Solanezumab increased CSF NfL levels. Correlation analysis showed stronger connections between CSF markers and fluid markers with Pittsburgh compound B positron emission tomography for solanezumab and placebo. The findings underscore the importance of reducing fibrillar amyloid in various Alzheimer's-related processes in DIAD, suggesting potential for non-amyloid biomarkers in disease modification.
