Among 1371 patients with high ischemic risk, de-escalation to clopidogrel demonstrated no significant difference in ischemic risk compared to ticagrelor-based dual antiplatelet therapy. Bleeding risks were consistent across both high and non-high ischemic risk groups. The findings suggest that the unguided de-escalation strategy with clopidogrel is comparable to ticagrelor-based therapy in stabilized AMI patients, irrespective of high ischemic risk.
