Early Cranial Ultrasonography Predicts Hypoxic-Ischemic Injury in Newborns

A retrospective cohort study assessed 149 newborns with hypoxic-ischemic encephalopathy (HIE), employing cranial ultrasonography (cUS) and post-rewarming brain magnetic resonance imaging (MRI). Severe HIE exhibited higher rates of abnormal white matter and deep gray matter hyperechogenicity on cUS than mild HIE. A normal cUS correlated with a 95% chance of normal/mildly abnormal brain MRIs. Severely abnormal cUS predicted 83% of severely abnormal brain MRIs. Higher cUS abnormality scores correlated with near-total brain injury on MRI. Severely abnormal MRI was an independent predictor of adverse outcomes. The study highlights cUS's predictive utility for early hypoxic-ischemic injury detection.
