The study aimed to investigate changes in biomarker levels related to inflammation and tissue destruction in the periapical exudate of mandibular premolar teeth with asymptomatic apical periodontitis after receiving intracanal cryotherapy. The study compared a cryotherapy group to a control group in terms of analgesic intake, interappointment, and postoperative pain and evaluated the correlation between biomarker levels and interappointment pain. The results showed a significant correlation between pain scores and IL-1β and PGE2 levels. Cryotherapy prevented an increase in IL-1β, IL-2, and IL-6 levels and reduced postoperative pain in the short term. The study suggests that biomarker levels can predict the severity of postoperative pain, and cryotherapy is effective in reducing pain.
