
This observational cohort study assessed the effectiveness of COVID-19 vaccination and prior infection against omicron infection in children under 12. Analyzing North Carolina residents aged 0-11, the study found primary vaccination effectiveness declined over time, with bivalent boosters more effective than monovalent. Omicron infection provided good immunity against reinfection, gradually waning. Vaccination and previous infection demonstrated better efficacy against severe outcomes (hospital admission or death) than infection alone. The study highlights the effectiveness of vaccines in children and informs preventive strategies for COVID-19.
