Efficacy and effectiveness of antipsychotics in schizophrenia

Researchers compared the efficacy of antipsychotics for schizophrenia relapse prevention using data from 90,469 individuals in Swedish and Finnish registries and 10,091 individuals from 30 randomized clinical trials (RCTs). Agreement in antipsychotic effectiveness was found between the registries. Real-world effectiveness was more significant in RCT-eligible individuals compared to RCT-ineligible ones. Efficacy in RCTs exceeded real-world effectiveness. Clozapine was identified as the most effective in the real-world network meta-analysis. Notably, long-acting injectable (LAI) antipsychotics performed slightly better in real-world settings. The study suggests RCTs might overestimate antipsychotic effectiveness in routine care, informing prescribing guidelines.
