Efficacy and Safety of Low‐Dose Edoxaban by Body Weight in Very Elderly Patients With Atrial Fibrillation

In the ELDERCARE‐AF trial subanalysis, low‐dose edoxaban (15 mg) demonstrated consistent benefits in elderly atrial fibrillation patients, even with extremely low body weight (≤45 kg). The study included 984 Japanese patients considered ineligible for standard oral anticoagulants due to bleeding risk. Edoxaban reduced stroke or systemic embolism in both weight groups (≤45 kg: HR 0.36; >45 kg: HR 0.31) but showed a numerically higher incidence of major bleeding (≤45 kg: HR 3.05; >45 kg: HR 1.40). Clinicians are advised to monitor the risk of major bleeding, especially gastrointestinal bleeding, in this population.
