Elderly Patients with Lumbar Spinal Stenosis and Sagittal Imbalance Face Poorer Surgical Outcomes, Study Reveals

A retrospective study investigated postoperative outcomes in elderly patients with degenerative lumbar spinal stenosis (DLSS) and severe global sagittal imbalance who underwent different fusion levels. Among 214 patients, 54 had sagittal imbalance syndrome and underwent long thoracolumbar fusion (Group A) or short lumbar fusion (Group B), while 30 without sagittal imbalance underwent short lumbar fusion (Group C). Patients with sagittal imbalance had more muscle degeneration and less potential for balance restoration. Results showed improved alignment and living quality postoperatively, but Group B had more proximal junctional complications. This suggests that DLSS patients with sagittal imbalance may have poorer outcomes with short fusion.
