
Etrasimod, a selective sphingosine 1 phosphate (S1P) receptor modulator, is approved for treating moderate to severely active ulcerative colitis (UC). Targeting S1P1,4,5 receptors without affecting S1P2,3, etrasimod demonstrated significant clinical remission improvements at 12 and 52 weeks compared to placebo in the ELEVATE trials. Efficacy was notably higher in biologic-naive patients and effective in cases with isolated proctitis. Etrasimod has a favorable profile with a shorter half-life, minimal drug and food interactions, and no dosing titration needed, contrasting with other S1P modulators like ozanimod. However, it is contraindicated in pregnancy, lactation, certain cardiac conditions, and skin cancer history. Etrasimod offers an effective oral treatment option for UC patients, particularly for those with moderate to severe disease and no prior biologic therapy.
