The study compared the mortality rates of COVID-19 patients treated with molnupiravir versus nirmatrelvir plus ritonavir during the Omicron variant era. The analysis used data from Italy's Medicines Agency database and the National Death Registry. Among 29,553 patients, molnupiravir-treated patients had a higher unadjusted mortality rate (51.83 per 100,000 person-days) than those treated with nirmatrelvir plus ritonavir (22.29 per 100,000 person-days). After adjusting for patient characteristics, the adjusted cumulative incidence of mortality at day 28 was 1.23% for molnupiravir-treated and 0.78% for nirmatrelvir plus ritonavir-treated patients. Nirmatrelvir plus ritonavir was associated with significantly reduced mortality risk, even in subgroups like fully vaccinated patients.
