A study compared the safety and short-term effectiveness of low-dose colchicine and oral prednisone for treating intense joint pain in older patients with acute calcium pyrophosphate crystal arthritis. The open-label trial involved 111 patients aged 65 or older. Participants were randomly assigned to receive either colchicine or prednisone. The primary outcome was joint pain reduction at 24 hours. Both drugs showed equivalent efficacy, with colchicine reducing pain by 36 mm and prednisone by 38 mm. Colchicine led to more cases of diarrhea (22% vs. 6% in prednisone group), while prednisone had higher incidences of hypertension and hyperglycemia. The study concludes that both drugs are equally effective in short-term pain relief for this arthritis, but have differing safety profiles in older patients.
