The study explores extracellular choline acetyltransferase (ChAT) activity, a key player in the cholinergic anti-inflammatory pathway. While cytoplasmic ChAT's neural role is established, this study unveils its presence and function in the extracellular space. The findings show that plasma ChAT responds to inflammation, increasing after immunological challenges. Direct activation of the cholinergic pathway boosts ChAT activity, inhibiting TNF production. Recombinant ChAT administration mitigates inflammatory cytokines and disease activity in an inflammatory bowel disease model. Elevated plasma ChAT levels in septic patients suggest its potential as an anti-inflammatory therapeutic agent, warranting further investigation.
