A prospective study conducted between 2021 and 2022 aimed to assess the feasibility and safety of using extracorporeal photopheresis (ECP) as a treatment approach for adult COVID-19 patients not responding to antiviral and immunomodulatory drugs. Seven patients with severe or critical COVID-19, unresponsive to standard treatments, were enrolled. Two ECP sessions per week for two weeks were administered. Results showed that among the survivors, there was no need for additional intensive care, radiological progression was limited, and viral clearance was achieved. ECP led to decreased levels of IL-6 and IL-17A and helped regenerate immune cells. The study suggests ECP as a promising option for non-responsive COVID-19 cases.
