A study examined the effect of fenofibrate on end-stage renal disease (ESRD) risk among patients already on statin therapy. The study compared 413,715 fenofibrate users with 413,715 non-users matched by age, sex, and triglyceride levels. Over a median follow-up period of 3.96 years, fenofibrate users exhibited a lower incidence of ESRD (0.885 per 1000 person-years) compared to non-users (0.960 per 1000 person-years). The hazard ratio for ESRD was reduced by 24% in fenofibrate users (HR 0.763, 95% CI 0.710–0.821). This association was particularly significant among patients with hypertension, proteinuria, or a reduced estimated glomerular filtration rate. These findings suggest that fenofibrate offers protective benefits against the progression to ESRD in patients on statin therapy.
