This retrospective study explored metronidazole-associated neurological adverse events through a cohort study and case-control analysis. Among 92,838 patients at Severance Hospital (2006-2021), 54 cases of metronidazole-related encephalopathy or neuropathy were identified. Risk factors included liver cirrhosis, chronic kidney disease, intravenous administration, and lower body weight. Results emphasize vigilance during prolonged metronidazole treatment in patients with these factors to detect potential neurological adverse events.
