Fresh-Frozen Allografts Show Superior Integration in ACL Surgery

The study investigated the biomechanical and histological integration of anterior cruciate ligament (ACL) allografts prepared via fresh-frozen (FF) and freeze-dried (FD) methods. In vitro analyses demonstrated that both graft types exhibited excellent biocompatibility and biomechanics, with the FD-allograft showing a denser collagen fiber arrangement. A canine model was used for in vivo evaluation, revealing no infection or osteoarthritis in the femur-graft-tibia complexes. However, the FF-allograft group displayed increased vascularity and synovitis. Micro-computed tomography showed significant bone remodeling in the FF-allograft group compared to the FD-allograft. Histological assessments indicated similar healing, but the FF-allograft group had significantly more robust integration, with a higher failure load observed (P < 0.05).
