In the AKINESIS study, 18% of acute heart failure patients with worsening renal function (WRF) developed adverse outcomes, including higher acute kidney injury stages, dialysis need, or death. Galectin-3, a cardiac biomarker, was found to significantly predict these outcomes, with an odds ratio of 4.6 for those in the highest tertile. Tubular injury biomarkers, however, were not associated with WRF, suggesting that WRF in acute heart failure is largely due to functional rather than structural mechanisms.
