Gastric Dysmotility Linked to PPI-Refractory Esophagitis in Systemic Sclerosis

A study investigated factors associated with proton-pump inhibitor (PPI)-refractory esophagitis in patients with systemic sclerosis. Out of 69 patients, 33% exhibited PPI-refractory esophagitis, while 67% had normal endoscopic findings. Univariate analysis revealed that patients with PPI-refractory esophagitis were more likely to have the diffuse subset of systemic sclerosis (43% vs. 17%, p=0.041). Gastrointestinal dysmotility, characterized by abnormal gastric emptying or small bowel dilation, was significantly associated with PPI-refractory esophagitis (66% vs. 8%, p<0.001). Analysis confirmed that gastrointestinal dysmotility (β=0.751, p=0.010) was independently associated with PPI-refractory esophagitis, whereas absent esophageal contractility and hypotensive lower esophageal sphincter were not significant
