Goal-Directed Rehab Improves Ataxia in Hereditary Cerebellar Ataxia

The multicenter randomized controlled trial investigated the impact of a 30-week goal-directed rehabilitation program on individuals with hereditary cerebellar ataxia (HCA). Participants were randomly assigned to receive rehabilitation or standard care. The primary outcome, motor function (measured by the motor domain of the Functional Independence Measure, mFIM), showed significant improvement at 7 weeks in the rehabilitation group (mean difference 2.26, p = 0.028). At 30 weeks, the rehabilitation group maintained improved ataxia (measured by the Scale for the Assessment and Rating of Ataxia, SARA) (mean difference −1.51, p = 0.017), but no significant difference was found in mFIM scores. These results suggest goal-directed rehabilitation improves function and ataxia severity, with sustained benefits beyond standard care.
