Histopathologic features of biologic therapy nonresponders in chronic rhinosinusitis with nasal polyposis

A study of 257 chronic rhinosinusitis with nasal polyposis (CRSwNP) patients undergoing functional endoscopic sinus surgery (FESS) revealed insights into biologic nonresponders. The study found that nonresponders to biologic therapies exhibited decreased tissue eosinophilia (<5 per high-power field) in 55% of cases, compared to 31.2% in controls. Additionally, 100% of nonresponders showed increased basement membrane thickening compared to 78.1% in controls. These histopathologic features suggest a non-type 2 CRSwNP that may resist biological therapies.
