A study analyzed histopathological differences between adult and elderly patients with chronic rhinosinusitis (CRS) to optimize medical management. The study included 300 adult and 77 elderly patients who underwent functional endoscopic sinus surgery. Adults had a higher prevalence of atopic status, while elderly patients had higher rates of comorbid diabetes mellitus. Adults showed more tissue eosinophilia and eosinophil aggregates, while the elderly exhibited more fungal elements and trends towards increased inflammation and tissue neutrophilia. The findings suggest that CRS in adult and elderly patients has distinct histopathological characteristics, and considering comorbidities can provide additional insights for optimizing management.
