This study investigated corneal higher-order aberrations (HOAs) and choroidal characteristics in myopic individuals, analyzing their associations with axial length (AL). Myopic participants were divided into three groups based on AL, and their spherical (Z40), comatic (Z31), and trefoil (Z33) aberrations, choroidal vascularity index (CVI), and choroidal thickness (CT) were compared. Longer AL was associated with reduced spherical aberration and horizontal coma. Additionally, CVI and CT values decreased with increasing AL. Linear regression analysis revealed that comatic (Z31) and trefoil (Z33) aberrations predicted mean horizontal CVI and CT. Findings indicate a potential relationship between corneal optical quality and ocular perfusion in myopic eyes.
