Human Histology after Structure Preservation Cochlear Implantation via Round Window Insertion

A study compared insertion techniques' effects on tissue formation. In one case using the round window (RW) approach, minimal trauma, and 10.8% fibro‐osseous tissue were observed. Cochleostomy (CO) and extended round window (ERW) cases showed more trauma and 30.6% mean tissue formation. In the basal 5mm of scala tympani, RW had 12.3% tissue formation, while CO/ERW had 93.8% mean formation. Findings suggest RW insertion can be less traumatic, supporting the hypothesis that cochleostomy drilling incites local tissue reaction.
