Hyperandrogenic PCOS Linked to Lower Pregnancy and Live Birth Rates

In a study involving 1,376 women with polycystic ovarian syndrome (PCOS), those with hyperandrogenic PCOS phenotypes demonstrated significantly lower pregnancy (29.9% vs. 40.2%, P=.02) and live birth rates (20.1% vs. 33.1%, P=.001) compared to those with nonhyperandrogenic PCOS. Despite similar age and race, the hyperandrogenic group had higher Body Mass Index  (BMI), fasting insulin, and insulin resistance. The odds of live birth remained lower in the hyperandrogenic group even after adjusting for the BMI-adjusted odds ratio (aOR 0.59). The study underscores the importance of considering distinct PCOS phenotypes in guiding individualized fertility treatment.
