ICS-Formoterol Reduces Asthma Exacerbations More Than SABA Alone

A network meta-analysis evaluated the efficacy of short-acting β agonists (SABA) alone, inhaled corticosteroids (ICS) combined with formoterol, and ICS with SABA for asthma management. Twenty-seven randomized clinical trials with 50,496 adult and pediatric patients were included. Both ICS-formoterol and ICS-SABA treatments significantly reduced severe exacerbations compared to SABA alone (ICS-formoterol: RR, 0.65; RD, −10.3%; ICS-SABA: RR, 0.84; RD, −4.7%) and improved asthma control scores (ICS-formoterol: RR, 1.07; ICS-SABA: RR, 1.09) with high certainty. In indirect comparison, ICS-formoterol reduced severe exacerbations more effectively than ICS-SABA (RR, 0.78; RD, −5.5%) with moderate certainty. Neither ICS-formoterol nor ICS-SABA increased the risk of serious adverse events.
