Impact of Decompressive Hemicraniectomy and WLST in Large Stroke Patients

In the SELECT2 trial, 352 patients with large ischemic cores and large vessel occlusion were assessed for decompressive hemicraniectomy (DHC) and early withdrawal of life-sustaining therapy (WLST). Of these, 55 received DHC, and 81 transitioned to WLST. No significant differences were found between endovascular thrombectomy (EVT) and medical management for DHC or WLST use. DHC was more frequent in younger patients, while WLST was more common in older patients. EVT maintained efficacy after adjusting for DHC, with 21% of DHC-treated patients achieving ambulation after 1 year. These findings suggest that DHC and WLST decisions did not influence the overall outcomes of the trial, indicating no bias in treatment allocation.
