
This cohort study investigated whether donor variables—sex, age, sex mismatch with recipients, and graft storage duration—affect clinical outcomes or failure rates in knee osteochondral allograft (OCA) transplantation. A total of 372 patients were followed for a mean of 5.4 years. Although a sex mismatch was more common in female recipients (90% vs. 10% for males), multivariable analyses revealed no significant impact of donor-recipient sex mismatch, donor age, or graft storage time on reoperation, failure rates, or achievement of clinically significant outcomes. These findings suggest that donor variables do not compromise clinical outcomes in OCA transplantation, challenging prior data.
