This observational case-control study explores familial influences on endometriosis characteristics. Among 344 patients, those with a family history (FH) of endometriosis exhibited higher rASRM scores and more endometriomas. FH of migraine correlated with reduced response to hormonal contraceptives. Depression in the family linked to milder endometriosis symptoms and better response to contraceptives but increased the risk of migraine and depression. Families with early menopause saw more severe endometriosis symptoms. Findings highlight distinctive traits in endometriosis patients with familial predispositions.
