This retrospective study examined the effect of intra-operative blood transfusion (IBT) on postoperative outcomes in elderly patients (aged 65 and above) with intertrochanteric fractures (IF) undergoing intramedullary fixation. After propensity score matching (PSM), IBT was found to significantly increase the incidence of postoperative delirium (POD) and hinder functional recovery. Although no significant differences were observed in length of hospital stay (LOS) or mortality, the findings suggest that IBT negatively impacts POD and postoperative functional outcomes.
