
The study investigates the impact of white matter hyperintensities (WMH), indicative of small vessel cerebrovascular disease, on cognitive outcomes in the Anti-Amyloid Treatment in Asymptomatic Alzheimer's Disease (A4) trial. Analyzing data from 1791 participants, the study reveals that increased WMH volumes in frontal and parietal lobes are linked to poorer cognitive performance, independent of amyloid positivity. The findings suggest the relevance of addressing small vessel cerebrovascular disease in Alzheimer's trials, emphasizing its potential role in influencing cognitive profiles and proposing it as a target for intervention or prevention.
