
Recent studies reveal the survival benefits of genomic profiling in guiding personalized therapy for metastatic castrate-refractory prostate cancer. The optimal timing for acquiring genomic profiles in the prostate cancer pathway remains unclear. This study focuses on men with aggressive disease (CPG 4–5 or de novo metastatic) at diagnosis, exploring genomic mutation yield through pre-biopsy imaging and guided biopsies. DNA damage repair mutations were found in 14%, including potentially actionable BRCA2 mutations. A pragmatic approach for molecular profiling at diagnosis within routine NHS pathways is presented, offering insights for personalized management.
