
This review discusses the management of inflammatory bowel disease (IBD) in pregnancy, including concerns about infertility, adverse pregnancy outcomes, and heritability of IBD. Effective disease management, pre-conception counseling, and multidisciplinary care can improve pregnancy outcomes. The safety of IBD therapies, including novel agents, is also discussed, with the majority being safe to use during pregnancy and lactation, with low risk to neonatal outcomes. The interplay between complex physiology in pregnancy and IBD can affect placental development, microbiome composition, and responses to therapy.
