This review examines the controversy surrounding the optimal timing of postoperative radiotherapy for patients with head and neck cancer. A comprehensive search of articles published between January 1995 and February 2022 yielded 23 relevant studies. Out of these, ten studies suggested that delaying postoperative radiotherapy could have a negative impact on patients, leading to a poorer prognosis. However, initiating radiotherapy four weeks after surgery did not appear to result in worse outcomes for patients with head and neck cancer. On the other hand, delays exceeding six weeks were associated with worsened overall survival, recurrence-free survival, and locoregional control. Therefore, it is recommended to prioritize treatment plans and optimize the timing of postoperative radiotherapy regimes.
